Frequently Asked Questions

Currently, patients may call and set up their own appointments with Dr. Cheng Tao and Dr. Derrick Cho without a referral from a physician. Spine & Neuro Center also offers appointments with our physician assistants and CRNP’s (Nurse Practitioners) without a referral from a physician. All other physicians in our practice require a referral from your primary care or urgent care physician. Please keep in mind that some insurance companies require referrals as well.

How do I get an appointment?

What do I need to bring with me?

  • All X-rays, MRI’s, and CT’s films pertaining to current problem/diagnosis

  • Insurance card

  • Driver’s license

  • Records regarding previous treatments

  • All medications

  • Co-payment or minimum payment (given at time of appt. for uninsured patients)

  • New patient registration forms

I am an established patient of a physician at Spine & Neuro, and I would like to see a different physician within the practice. What should I do?

For continuity of care, our physicians retain the same patients. However, special concessions may be made in individual cases.

I have been seen at Spine & Neuro in the past. Do I need another referral?

If you are already an established patient, it may be necessary to get another referral if:

  • You need to be seen for a different problem / diagnosis

  • If your insurance requires a new referral
